OSHL Inc. Terms and conditions

The player assumes all risk of personal injury which may result from participation in all activities of the Original Six Hockey League (OSHL Inc.). The player will not hold OSHL Inc. or any of OSHL Inc.’s officials, representatives, owner or proprietor or employees of any ice facility used by the OSHL Inc., liable for injury which the player may sustain while participating in any OSHL Inc. activity. The player understands and agrees that the sport of ice hockey has physical dangers which may result in personal injury, illness, permanent disability and death. The player is advised to carry medical insurance. The player certifies that he/she has no known medical condition which would prohibit him/her from playing the sport of ice hockey.

The OSHL Inc. reserves the right to accept/decline registrations. Aggressive players may be asked to leave the league at any time without compensation. Team Jerseys are loaned to you for the season and must be returned. Any unreturned jerseys or damage caused to them deemed outside of normal wear and tear will be billed back to you. There are no refunds.

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19

The player acknowledges the contagious nature of COVID-19 and voluntarily assumes the risk that he/she may be exposed to or infected by COVID-19 by participating in any OSHL Inc. activities, and that such exposure or infection may result in personal injury, illness, permanent disability and death.

I hereby release, covenant not to sue, discharge, and hold harmless the OSHL Inc., its officials, representatives, owner or proprietor or employees of any ice facility used by the OSHL Inc., of and from the claims, including all liabilities, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims related to the OSHL Inc.’s officials, representatives, owner or proprietor or employees of any ice facility used by the OSHL Inc., whether a COVID-19 infection occurs before, during, or after participation in any OSHL Inc activity.

I agree to follow the guidelines outlined by OSHL Inc. and those of any ice facility used by the OSHL Inc. Please acknowledge that you have read the above waivers and agree with the terms and conditions by submitting this registration.
